How to Fill Teacher Evaluation Form on GULMS

Step 1: Go to http://gulms.live

Step 2: Click On Google Button. How to Integrate Google Sign-In into University LMS

Step 3: Enter your University Email Address and password

Step 4: Click on the Course

Step 5: Click on the Fall 2021 - Teacher Evaluation Form Link

Step 6: Click on Answer the Questions

Step 7: Give answer to 20 Questions Performa

Step 8: Click on Submit of Answer Button.

Do step 4 to Step 8 for all the courses.
